Photograph, Video, Name and/or Quotation Release

I, the undersigned, hereby give permission to The MindFuel Foundation to use my name, image or quotation in its public relations, new funding proposals, reporting, and communication materials created in perpetuity from date consent is signed. I realize that I may withdraw my consent in writing at any time by contacting The MindFuel Foundation or by visiting My Profile Page and opting out of the media waiver.

I understand that the project video submission(s), online event recording(s), photograph(s), video footage, and/or quotation from Connect 2 Innovate and/or interviews, articles or other social media about the Connect 2 Innovate may be used in a publication, print advertisement, direct-mail piece, television broadcast, electronic media (e.g., video, CD-ROM, internet, World Wide Web, Social Media, etc.) or other forms of communication in a professional manner.

All communications by The MindFuel Foundation where this material will appear shall constitute the property of The MindFuel Foundation, solely and completely. I hereby relinquish any and all personal or proprietary rights I may have in connection with such use. I understand that I will receive no compensation should any of this material be used. I realize that I may request access to communication at any time by contacting The MindFuel Foundation. 

In giving my consent, I hereby release and hold harmless The MindFuel Foundation and their partners, agents, employees, officials, representatives and contractors from any and all responsibility or liability for damage of any kind suffered in any manner whatsoever.

I understand that because this material will be available to the public in The MindFuel Foundation’s communications and/or reporting, it is not possible to consider an expiry date for this consent. Cancellation of my consent may only limit the use of my information in the future or new publications.
